What actually causes this on a VAG

The position signal from the exhaust flap is reading higher than a working sensor could produce, or the circuit is reading as open. In practice that most often means the actuator is unplugged, the signal or earth wire is broken, or the signal wire is touching a supply voltage somewhere. A disconnected actuator will often set this code.

Most likely causes, in order

  1. Connector unplugged, not fully seated or damaged. The classic after any work on the turbo, downpipe, particulate filter or exhaust - the plug gets knocked off or pushed on without latching. Confirm by physically checking that it is home and the locking tab has clicked, and looking for a terminal pushed back inside the housing.
  2. Broken signal or earth wire. An open circuit in either will send the reading to the top of its range. Confirm with a continuity check along the wire from the actuator plug back towards the ECU with the ignition off, flexing the loom as you go.
  3. Signal wire shorted to a supply voltage. A lead that has rubbed against a live feed pins the signal high. Confirm by disconnecting the actuator and measuring the loom-side signal pin with the ignition on: a supply voltage sitting there - whether the sensor reference voltage or battery voltage - points at a chafed wire somewhere, not a bad sensor.
  4. Failed position sensor inside the actuator. Reached last, once the plug and the wiring have both been proven good. On electric actuators the sensor is normally integral, so check the parts list for your car before assuming a sensor can be bought on its own.

Worth knowing: If this appeared immediately after exhaust, turbo or particulate filter work, the overwhelming likelihood is a plug that was not pushed fully home or a lead pinched during refitting. Check that before anyone orders a part - it costs nothing to look, and it is the most common way this code turns up.
